Notes:
- Example names and place names must be replaced with the actual name appropriate to the person being addressed.
- The written address is only meant for letter envelopes. Within an actual letter, the form used for salutation should be utilized.
- For several positions, no special oral appellation or salutation is used. Holders of these positions may be referred to by their private titles (i.e., Mr Smith, etc.), or by their positions.
- For most judges, the surname is used along with a title such as "Judge" or "Mr Justice."
- When two or more judges share the same surname, the senior judge uses the surname alone (Judge Smith), while the junior ones use the forename and the surname (Judge John Smith.)
- If a judge is a peer or peeress, then the title of peerage is included in the title. The form is "Judge The Lord Smith" or "Judge The Lady Smith."
- If a judge is a knight or dame, then the full name and appellation of knighthood are used. The form is "Judge Sir John Smith" or "Judge Dame Jane Smith."
